This academic research has been approved by two US universities' Institutional Review Boards (IRB) and it poses no risks to respondents. Nice survey. Asks too many personal questions at the end though, IMO. (I skipped over those.) I'm guessing that the student author of this survey will use it as part of a Doctoral thesis. I conducted a survey similar to this as part of a Masters degree many moons ago (but not sports-related, obviously).
